Eskenazi Health Center Forest Manor has been designated an Age-Friendly Health Systems Participant by the Geriatrics Workforce Enhancement Program, a grant program under the Health Resources & Services Administration, for integrating age-friendly care into the primary care environment. Age-friendly practices are based on implementation of the four Ms, mentation, medication, what matters most and mobility, which are essential elements to the care of older adults.

Eskenazi Health Center Primary Care – Adolescent Program
Recognizing that teens have distinct and diverse needs, Eskenazi Health Center provides comprehensive health services for them – from routine health care to treatment of emergencies and chronic illnesses. Services include care for acne, sports-related injuries, menstrual issues and general illnesses in addition to mental health screenings, smoking cessation programs and immunizations.
