- Access your child's growth charts, immunization history and upcoming appointments, if applicable
- Early check-in for your appointment
How To Sign Up
Eskenazi Health patients can sign up online. If you recently visited an Eskenazi Health location, please look for your MyChart activation code on your After Visit Summary. If you do not have an activation code, you can create an Eskenazi Health MyChart account by entering information that matches your medical records.
Follow the below link, look for the New User section and click Sign Up Now. If you don’t have an activation code, choose the Request Online option near the bottom of the page. You can use the Quick Start Guide to help you get started.
