Eskenazi Health Center Discounted Services Notice to Patients

Senior Primary Care Services
On Friday mornings from 8 a.m. - 12 p.m., Eskenazi Health Center Barton Annex has a geriatrician available to provide primary care services to the residents of John J. Barton Annex and John. J. Barton Tower as well as older adults in the community. These services are for patients 65 and older who would like to establish care with a primary care provider who specializes in the care of older adults.

No referral is needed for senior primary care services at Eskenazi Health Center Barton Annex and walk-ins are welcome. Reasons for an appointment may include:

  • Complete primary care services for older adults
  • Functional loss/frailty
  • Home safety
  • Incontinence
  • Malnutrition/weight loss
  • Medication management
  • Memory concerns
  • Mobility issues/falls
  • Abuse/mistreatment
  • End-of-life planning
