At Eskenazi Health, more than 60 financial counselors – including certified application counselors for the Health Insurance Marketplace – are available to help patients determine the method of payment and/or insurance plan that is best for them.

Eskenazi Health financial counselors can complete the following applications:

  • Hoosier Healthwise
  • Healthy Indiana Plan
  • Medicaid: for aged, blind or disabled individuals
  • Health Advantage
  • Federal Marketplace

Financial counselors also can give referrals for other payor resources to qualified patients. Counselors are available at the Sidney & Lois Eskenazi Hospital and at Eskenazi Health Center and Eskenazi Health Midtown locations.

What to Bring to a Financial Counseling Visit

Below is a general list of items patients may need during the application process. For questions about whether a document is necessary for a specific application, please call a financial counselor at 1.855.202.1053, choosing option 1.

  • Valid state-issued photo operator’s license or ID card
  • Birth certificate
  • Social Security card
  • Copy of Medicare, Medicaid/HIP card or award letter
  • Proof of Marion County residency, for example: utility bills, mortgage, current lease
  • Statement of support from person(s) providing room and board with his/her name, address and description of services being provided
  • Vehicle title and/or registration
  • Life insurance policies
  • Proof of dependents, for example: Social Security card or birth certificate, tax records with dependents listed, court-ordered guardianship or custody papers, documentation of pregnancy
  • Additional information, for example: current food stamp award letter, current trustee assistance – poor relief award letter, current homeless shelter residency verification
  • Proof of income, for example:
    • Paycheck stub from past three months plus current month or current statement from employer on company letterhead with current gross income
    • Current statement from INET (WorkOne) if unemployed
    • Social Security or SSI/SSDI award letter (call 1.800.772.1213 for replacement card)
    • Past two years’ annual tax returns
    • Verification of pension or retirement benefits
    • Last three months of complete bank statements for all accounts
    • Verification of student loans or grants
    • Divorce decree
    • Verification of any other income
    • Receipts of child support payments
