Eskenazi Health Center  

Through a network of primary care facilities located throughout the neighborhoods of Indianapolis, Eskenazi Health Center, a federally qualified health center, is comprised of 10 separate health care sites.

Eskenazi Health Center includes the following locations: Eskenazi Health Center Blackburn, Eskenazi Health Center Cottage Corner, Eskenazi Health Center for Senior Health, Eskenazi Health Center Forest Manor, Eskenazi Health Center Grassy Creek, Eskenazi Health Center Mobile Clinic, Eskenazi Health Center North Arlington, Eskenazi Health Center Pecar, Eskenazi Health Center Primary Care and Eskenazi Health Center Westside. These sites offer an array of services, including physical exams, prenatal care, social services, dental services and more.

Physicians working in the Eskenazi Health Center sites also serve as faculty physicians of the IU School of Medicine.

Open Access Scheduling

Eskenazi Health Center follows an open access scheduling model, allowing patients to make same-day, primary care appointments. Instead of scheduling appointments weeks in advance, after a primary care appointment, patients are given a card with the timeframe their doctor would like to see them again and are asked to call on the day they are able to come in for their appointment. This is backed up with a computer registry which tracks patients’ due dates and actively reaches out to patients to remind them when it’s time to come in again.

“Pre-booked” appointments (up to 30 days in advance) are available for patients who have special needs or need to coordinate their care with others. To make either type of appointment, please call 317.880.5555 or your Eskenazi Health Center site.
